N. Daniels의 생애에 대한 타산적 접근법과 그에 대한 비판: S. Brauer를 중심으로

이 논문은 연령주의(Ageism) 즉, 역연령(Chronological age)을 기준으로 한 보건의료 자원 분배논의를 주제로 하여 N. Daniels의 생애에 대한 타산적 접근법(Prudential lifespan approach)과 이에 대한 비판을 살펴보고, 이를 통해 이론이 지닌 한계를 개선시키고 현실적 상황에 대입할 수 있는 지점을 탐구하는 것을 목적으로 한다. 생애에 대한 타산적 접근법은 연령에 따른 의료 자원 분배 논의를연령 집단간 경쟁이 아닌 개인의 생애 전체에서의 연령별 분배로 바라보게 한다. 하지만 이 이론을 통한 N. Daniels의 연령주의 정당화가 일반적인 상황에서는 이루어지지 않으며 연령별 분배를 정당화하는 과정 또한 논리적 결점이 존재한다. 이러한 약점들을 구체적으로 S. Brauer와 M. Schefczyk가지적하고 있으며 이는 철학적인 문제뿐만 아니라 현실적 제약까지도 포함하고 있다. 필자는 이러한 한계에도 불구하고 생애에 대한 타산적 접근법이 지닌 강점이 분명히 존재하고, 이를 통해 연령별 분배논의가 더욱 합리적으로 지속될 것이라 전망한다. 이를 위해서는 연령별 분배에 대한 사회적 논의가필요하고 건강보험 재정의 지속 가능성에 대한 개선이 선행되어야 할 것이다.

Norman Daniels proposed the prudential lifespan approach (PLA) to resolve debates about ageism. In this approach, he redefined the allocation of healthcare resources, especially regarding life-sustaining treatment, between age groups and tried to change the focus of the ageism debates from competition between the young and the old to personal allocative decisions within one’s lifespan. Using this approach, Daniels justified rationing healthcare resources by age in some limited settings. However, several critical problems remain with the justification of age-based rationing made by PLA. Brauer and Schefczyk have pointed to philosophical as well as practical problems with the PLA. This article examines both the PLA and the critiques of it offered by these two authors. While maintaining the basic idea of the PLA, several modifications to the justification of age-based rationing using PLA are made in order to solve these both the practical and philosophical problems. Furthermore, recommendations for the application of this modified justification of age-based rationing in a real-world setting are given.
